Birth records may be requested by the person named on the record if they are over the age of 18, their parents or a legal representative. Applicants should bring the completed Application to Local Registrar for Copy of Birth Record form, along with proper identification, proof of need, and the appropriate fee to Town Clerk's office. Please make checks or money orders payable to Mark Bonilla, Town Clerk.

  7. Two utility bills, showing applicant's name and address
